Christian Community Home of Osceola’s long-term care program is designed for residents whose health conditions are complex and/or medically unstable and who require frequent medical or nursing intervention. We offer our residents many choices when it comes to their daily lives. Should your loved one enter the care of CCH, he or she along with the physicians, care team, and your family will work together to formulate an individualized plan of care. This comprehensive approach is designed to address not only his or her health needs, but functional abilities, chronic illnesses, and desired outcomes.
